The period following hospital discharge can be difficult and challenging for patients and their families. Often times the lack of proper continuity of care may increase the risk of relapse and/or readmission. For these reasons we provide nursing care that caters specifically to post-hospitalized patients.  We offer comprehensive care which encompasses everything from antibiotic administration to wound care, physical and occupational therapy.
